Your First Chiropractic Visit Includes...

New Patient Intake Forms
Upon entering the office, we have some brief paperwork for you to fill out. The paperwork provides us with general information about you and your condition. You may also download the intake form below to print and fill out at your convenience in the privacy of your own home. Please bring the completed form with you on your first visit.

Consultation
Once the paperwork has been completed, you will have a consultation with the doctor to discuss your health-related issues and concerns, and discuss potential treatment options.

History and Examination
In order to determine what your actual problem is, you will be asked various questions related to your overall health and history of spinal impacts, including auto-accidents, work, recreational and sport injuries. Next, a number of specialized tests will be performed to determine the health of your nervous system and if you are suffering from the Vertebral Subluxation Complex.

X-ray Studies
Your specific condition may require us to take x-rays to either rule out more serious conditions or assist us in developing the most effective care plan for you. The x-rays reveal the degree of spinal misalignment and the phase of spinal degeneration. These findings enable us to provide you with individualized treatment and optimal adjustments.

Schedule Your Next Appointment

Upon completion of your examination you will be asked to schedule two consecutive appointments.  The first appointment will provide you with a complete review of your exam findings.  At that point, should the doctor determine that they can accept your case you will receive your first adjustment.   At your second visit, based on their findings, the doctor will make their best recommendation for care and you will be presented with a plan designed to provide you with optimal results.
